Email Orders:
You can copy and paste the order form page in an email and fill in all the necessary information. Please DO
NOT input credit card information on your email. If you wish to pay by credit card we will contact you back within 24
hours of receiving the email to verify payment information on your order.
Mail Orders:
Go to the order form page and print the form. Fill in the complete order form making sure to include the
item description, size, price and quantity. Please be sure to provide your street address (no P.O. Boxes) and your payment
information. Mail your order to:

Fax Orders:
Go to the order form page and print the form. Fill in the complete order form making sure to include the
item description, size, price and quantity. Please be sure to provide street address (no P.O. Boxes). Our Fax
is secure, you may include your payment information on the order form when faxing.

Method of Payment:
Payment is due upon placement of your order. Credit card (VISA or Mastercard) is the
preferred method of payment. If you wish to pay by check, we will accept this method of payment,
but reserve the right to to ship after the check clears. There is a $35 fee for returned
checks.
Shipping:
Due to the variable volume of orders, we request you verify the shipping date when you place an order.
We
strive to ship the Jonart Whelping Boxes within 1 week maximum. Please check the
Jonart News/Show Info page to view current lead times. Shipping & Handling Costs can be found on the
Order Form page. Prices for shipping are based on UPS Ground to anywhere in the Contiguous United States. For Hawaii,
Alaska or International orders, call or email for quote on Shipping. Be sure to include the items you wish
to purchase for shipping and your shipping address including postal code/zip code.
International Orders:
Orders can be placed either by telephone (1-419-266-2509), fax (1-419-720-8078) or mail to: Jonart Whelping Box
Co., Inc., 2716 Centennial Road Toledo, Ohio 43617. Visa and Mastercard are the accepted methods of payment. All
duties and taxes are the customers's responsibility and are collected by the shipping company (UPS, or other).
Returns:
Returns will only be accepted with our prior approval, so please contact us via phone first.
All claims must be made within (10) business days and returned to us within (14) business days. Returned
items will be issued a refund of the product cost only and minus a $25 restocking fee for Small &
Medium Boxes and $15 for sm/med. pens and a $50 restocking fee for Large and XL Boxes and $25 for Lg/Xl pens.
The restocking fee's for boxes are based on complying with the 10 business day notification and returned within 14 business
days. All other considerations will be handled on a case by case basis. There is no return opportunity for used
boxes! For returned fleece, assuming it was never used, there is a $10 restocking fee per
box.
